Dear Parents,
Below are links to our registration forms for the upcoming school year. If you are interested in enrolling your child for next year, please return it with your registration fee as soon as possible.
We are required by the state to have all of the information completed. Make sure all of the forms are filled in.
If you would like to bring your child in for a visit, please call 419-866-4937 to set up an appointment time.
We will begin accepting registrations February 1.
